NHS Flu Vaccination Guide

 

Thank you for choosing to have your flu vaccination with us this year. There will be some changes to how we perform our vaccinations this season to ensure both the safety of our staff and yourselves. We will be making appointments this year with adequate spacing to ensure appointments do not overlap.

Before Appointment

  1. Please complete the Consent Form with Name, Address, Signature and Date.

 

  1. Please complete the entire first page (p15) of ‘Annex E NHS Flu Vaccination Service - Record Form’. The back page (p16) will be completed by the pharmacist on the day.

 

  1. The Pharmacy will call you to conduct a Covid-19 pre-screening questionnaire.

 

At Appointment

  • Please bring in both forms to your appointment

 

  • Come to the appointment alone if possible

 

  • Wear a short sleeved top and please avoid bringing bags and coats where possible

 

  • Wear a face mask or covering unless exempt

 

  • You will be asked to sanitise your hands before and after the vaccine so please bring your own if you do not wish to use ours.

Do not come in for your appointment if:

  • You or anyone in your household have a high temperature, new continuous cough or loss or change in sense of taste or smell.

 

  • You have had recent, close contact with anyone who is now self-isolating as a result of possible or confirmed COVID-19
